- Pain on one side of the face: Irritation usually affects just one of the trigeminal nerves (right or left), so pain localizes to the areas served by that branch.
- Triggered by touch, chewing, or talking: Nerve damage can cause painful sensations in response to stimuli that should otherwise be painless. An irritated trigeminal nerve can fire up in response to mild sensations, such as brushing your teeth, shaving, applying makeup, or even touching your lip with the tongue.
- Episodes worsen over time: Ongoing nerve damage increases sensitivity, so attacks may become more frequent, severe, and longer-lasting.
- Constant aching or burning: Chronic irritation of the trigeminal nerve can cause a background pain that is perceived between attacks. This occurs because the irritated nerve sends persistent, low-level pain signals.
Pain attacks may be triggered by mild stimuli, such as contact with the cheek, vibration, or touch, such as washing your face, eating, or drinking. Understanding what triggered a previous attack and avoiding that trigger may help you prolong a period of remission.

While several factors can irritate or damage the trigeminal nerve, most cases fall into four primary categories: neurovascular compression, demyelinating and neurological disorders, structural or space-occupying lesions, and traumatic, infectious, or idiopathic factors.
1. Neurovascular Compression
Neurovascular compression is the most common cause of trigeminal neuralgia. It happens when a blood vessel (usually an artery) that runs nearby presses against the trigeminal nerve near where it enters the brainstem.
The blood vessel causes constant pressure that irritates or damages the nerve’s protective sheath (called myelin). This makes the nerve “short-circuit” and send painful signals even due to mild triggers that should normally not be painful.
Vascular compression may affect the trigeminal nerve in different ways:
- Superior cerebellar artery compression: The superior cerebellar artery travels between the brainstem and the trigeminal nerve. Its close position makes it the vessel most often responsible for direct nerve compression.
- Anterior inferior cerebellar artery compression: The anterior inferior cerebellar artery can form a loop that crosses near the trigeminal nerve root, potentially compressing the trigeminal nerve.
- Other abnormal arterial loops near the trigeminal nerve: Sometimes, other arteries located near the trigeminal nerve form unusual loops or twists that pinch or press on the nerve, especially as blood vessels change shape with age.
2. Demyelinating And Neurological Disorders
Demyelination refers to the loss or damage of myelin, which is the protective sheath around nerves. The myelin plays a major role in ensuring that electrical signals travel smoothly between the brain and different areas of the body.
When myelin is lost or damaged, nerve signals become erratic or abnormal. This increases the nerve’s sensitivity, causing it to misfire and causing pain even in response to stimuli that should be painless.
Demyelination can occur due to repeated irritation or compression of a nerve. It can also be caused by neurological diseases that involve inflammation or immune attacks on nerves. These include:
- Multiple Sclerosis (MS): MS is an autoimmune condition where the immune system attacks myelin in the brain and spinal cord. It can sometimes affect other nerves, including the trigeminal nerve. Research estimates that 15% of individuals with trigeminal neuralgia are later diagnosed with multiple sclerosis, and up to 3.4% of people living with MS experience trigeminal neuralgia.[3][4]
- Other demyelinating disorders (rare): Other diseases can also damage the myelin.
- Acute Disseminated Encephalomyelitis (ADEM): ADEM is a brief but widespread attack of inflammation in the brain and spinal cord, sometimes caused by infection. The intense inflammation can strip myelin from nerves and lead to TN symptoms.
- Neuromyelitis Optica Spectrum Disorder (NMOSD): NMOSD is an immune disorder targeting the optic nerves and spinal cord, but it can also cause demyelination in facial nerves.
- Chronic Inflammatory Demyelinating Polyneuropathy (CIDP): CIDP is a nervous system disorder where chronic inflammation leads to a gradual loss of myelin.
- Leukodystrophies: These are inherited disorders that affect myelin formation, which can disrupt nerve signaling throughout the brain. They may trigger facial pain if the trigeminal nerve is involved.
3. Structural And Space-Occupying Lesions
Structural lesions and tissue growths that crowd the surrounding space (such as tumors) can press on the trigeminal nerve. These include:
- Acoustic neuroma (vestibular schwannoma): This is a benign tumor that develops on the nerve responsible for balance and hearing. Its location near the trigeminal nerve can lead to direct compression.
- Meningioma: This is a typically benign tumor in the membranes covering the brain and spinal cord. When this occurs near the trigeminal nerve, it can increase pressure and trigger TN symptoms.
- Epidermoid cyst: This is a slow-growing, noncancerous cyst in the brain that may press against the trigeminal nerve or its root near the brainstem.
- Arteriovenous malformations: These are tangles of abnormal blood vessels that can put pressure on nerves, including the trigeminal nerve.
- Cerebellopontine angle tumors: These tumors grow in the space between the cerebellum and the pons, an area where several important nerves—including the trigeminal nerve—pass through, potentially causing compression.
- Aneurysms pressing on the trigeminal nerve: Aneurysms are balloon-like bulges in an artery. When these occur near the nerve can physically compress the trigeminal nerve and result in TN symptoms.
4. Traumatic, Infectious, And Idiopathic Causes
Trigeminal neuralgia can also arise from trauma, certain infections, or for reasons that remain unclear (idiopathic). Injury, inflammation, or infection can damage the trigeminal nerve directly or indirectly, causing it to misfire and leading to facial pain.
In some cases, the exact cause is never found. This is common in older adults as nerves naturally degenerate with age.
- Facial trauma: Blunt force to the face or jaw can physically damage the trigeminal nerve or nearby structures.
- Dental procedures or surgery affecting the nerve: Dental work, oral surgery, or tooth extractions can sometimes irritate or damage a branch of the trigeminal nerve.
- Herpes zoster infection (shingles): In some cases, the reactivation of the virus that causes chickenpox (Herpes zoster) can inflame nerves in the face, including the trigeminal nerve. This condition is called postherpetic neuralgia.[5]
- Sarcoidosis (rare): This is an inflammatory disease that can produce clumps of immune cells, called granulomas, in the nervous system. These can potentially press on or affect the trigeminal nerve.
- Lupus (rare): This autoimmune disorder causes widespread inflammation, sometimes damaging the nerves’ myelin.
- Idiopathic or age-related nerve degeneration: In many cases, particularly in older adults, TN occurs without a clear reason, possibly due to gradual nerve wear or unknown factors. Generally, women are affected twice as much as men, and it is most commonly diagnosed in adults between 37 and 67 years old.[6]
