Post-concussion syndrome (PCS) is a condition that can disrupt nearly every aspect of daily life. After an initial head injury—such as a blow, fall, or accident—most people expect typical concussion symptoms like headaches or dizziness to fade within weeks.

During a concussion, the brain shifts within the skull, which can cause tissue damage and temporarily disrupt normal brain function. 90% of concussion symptoms, such as headaches and dizziness, resolve within 10 to 14 days as the brain begins to recover and return to normal function.[1]

However, in some cases, symptoms continue or return weeks or months later—this is known as post-concussion syndrome (PCS). As many as 43% of people who have a concussion develop PCS, which is diagnosed when symptoms such as fatigue, headache, depression, and trouble concentrating last for three months or more.[2]

PCS develops when the brain and related systems are unable to return to their previous state after the initial trauma. Not only does this prolong the healing process, but it can also cause the development of new symptoms that were not present immediately after the injury. 

Common signs and symptoms of post-concussion syndrome can include:

  • Ongoing headaches or migraines
  • Dizziness or problems with balance
  • Sensitivity to light or noise
  • Difficulty concentrating or remembering things
  • Feeling mentally “foggy” or slowed down
  • Changes in mood, such as irritability, anxiety, or depression
  • Sleep problems, including trouble falling or staying asleep
  • Fatigue, even after rest

What makes PCS complex is that symptoms may stem from a combination of factors, including underlying injuries to the neck or spine that were not diagnosed after the injury. Because these contributing issues can be subtle or delayed, a careful and comprehensive diagnosis becomes essential for any person who experiences ongoing problems after a concussion. 

How Long Does It Take To Recover From PCS?

The timeline for recovering from post-concussion syndrome (PCS) can look very different for each person. Typically, PCS is diagnosed when symptoms last three months or longer. 

However, some symptoms can last beyond this timeframe. For instance, after a head injury, 90% of people experience headache, a common concussion symptom that occurs due to brain tissue damage and inflammation. However, about 30% of people who have experienced a concussion still suffer from headaches at six months, and nearly 58% experience them a year or more after the injury.[3]

Factors like severity of the initial injury, pre-existing health, and whether there are undiagnosed complications—such as injuries to the upper neck—influence how long recovery takes. 

Mild Cases

In mild cases, PCS symptoms are usually manageable. They often include headaches, fatigue, or mild difficulty concentrating. Most people who have experienced a mild head trauma start to see improvements within a few weeks. Treatment may include rest, gradual return to activities, quality sleep, and basic self-care.

Moderate Cases

Moderate PCS may lead to more persistent or disruptive symptoms that interfere with work, school, or daily life. These may include memory problems, frequent headaches, or emotional changes.

Severe Cases

Severe PCS may be caused by a more significant head trauma. These cases are more likely after a severe concussion or multiple concussions, or if underlying issues such as craniocervical instability are present. Recovery in these scenarios often requires specialized diagnostics and a customized, comprehensive treatment plan.

CCI is damaged to the deep internal upper neck ligaments, such as the alar, alar accessory, transverse, apical, ALL, AAOM, PAOM, sAOOL, tectorial membrane, etc., which leads to a multitude of symptoms due to excess stress on upper neck structures. CCS is an upper neck facet joint, nerve, tendon, or muscle injury that leads to symptoms but no significant ligament injuries, causing instability. These injuries often develop from the same trauma that causes a concussion. However, because their symptoms overlap with PCS—like headaches, dizziness, brain fog, and visual disturbances, they can go unnoticed if only standard evaluations or diagnostic tools are performed.

Comprehensive Diagnosis With Advanced DMX Imaging

Traditional imaging methods often miss injuries to the upper neck because these problems only appear when the head and neck are in motion. 

That’s why Centeno-Schultz Clinic CSC leverages state-of-the-art technology like Digital Motion X-ray (DMX)—a dynamic imaging technique that captures real-time movement of the upper neck, joints, and ligaments. Unlike regular X-rays or MRI scans that take static images, DMX reveals hidden ligament injuries, joint instability, and misalignments. CSC does hundreds of these procedures a year without complication. CSC uses high-dose, highly concentrated platelet-rich plasma (PRP) and or bone marrow concentrate (BMC) containing stem cells to help to heal these joints, slow arthritic progression, which can in turn reduce pain and improve function. For patients with CCI who have deep internal upper ligament damage, such as the alar, transverse, apical ligaments, etc., CSC has developed the PICL procedure to precisely inject those ligaments to improve stability and reduce stress on the upper neck joints, nerves, blood, CSF flow, and muscles. The PICL is a novel procedure developed and only performed at CSC. For this procedure, you have to inject through the back of the throat to safely reach those internal ligaments. This requires a CRNA anesthetist, 2 C-ARM fluoroscopy units, a laryngeal scope, and a special 3D printed mouthpiece to depress the tongue and open the airway.
